Understanding your Medical Plan Options

Option 1: Anthem HDHP with Garner
Premium: $0 for ALL coverage tiers
(Employee Only, Employee + Spouse, Employee + Child(ren), Family)


This continues to be 100% employer-paid—a premium benefit at no cost to you. Plus, when you use Garner-approved providers, you’ll have no out-of-pocket costs, meaning $0 for premiums, deductibles, co-pays, or coinsurance! It's all covered!


What is Garner?
Garner is a high-quality, high-deductible health plan that helps you find the best doctors and save money. Their concierge team does the heavy lifting:

  • Researches doctors for you based on quality and cost

  • Books appointments on your behalf

  • Negotiates bills when needed

  • Provides a Garner Health Card with savings on prescriptions and services

Best for:  Employees who want high-quality, personalized care and low to $0.00 out-of-pocket costs—and are willing to take a few extra steps to get there. Garner rewards you for choosing top-performing doctors, which can mean $0 medical premiums when you follow their guidance and stay engaged in your care.
Option 2: Anthem EPO
Employee Only Premium: 100% employer
Dependent Premium: 85% employer | 15% employee


Members are required to use healthcare providers within the plan's network to receive coverage. This Plan is available nationwide and no referrals are needed. Except for emergencies, care received outside of the network isn’t covered.

Best for:
Employees who want lower premiums and don’t mind staying within a strong national network. Great for those who prefer simplicity—no referrals needed, predictable costs, and in-network access to quality care.

Option 3: Anthem PPO
Employee/Dependent Premium: 75% employer | 25% employee


Members have the flexibility to see any provider-both in and out of network, but you will have lower out of pocket costs if you stay in network.  This plan provides a lower deductible, predictable copays for routine care and broad nationwide access.

Best for:
Employees who want the most flexibility and choice in where they receive care, and don’t mind paying slightly higher premiums for that freedom. Ideal for those who travel frequently or have preferred providers outside the network.

Note for current PPO members: Your premium contribution is increasing. We strongly encourage you to evaluate Garner or HDHP plans—you may find they meet your needs at a significantly lower cost.

Option 4: Anthem HDHP with HSA
Employee Only Premium: 100% employer
Dependent Premium: 85% employer | 15% employee


Members are enrolled in a lower-premium, high-deductible plan paired with a tax-free Health Savings Account (HSA). They can use HSA funds for eligible expenses, save for future care, and keep any unused balance year to year—even if they leave GoFundMe.


Best for:
Employees who want lower premiums, prefer to save for future medical expenses, and like more control over how their healthcare dollars are spent. Great for those who rarely need care or want to build long-term savings through the tax advantages of an HSA.

Option 5: Kaiser HMO - Available to CA residents Only

Employee Only Premium:  100% employer
Dependent Premium: 85% employer | 15% employee

Members are required to use Kaiser facilities and providers to receive coverage. This plan is available only to California residents and offers care that’s fully coordinated within Kaiser’s integrated network. Referrals are required to see specialists, and out-of-network care isn’t covered except in emergencies.


Best for:
Employees who want predictable costs, coordinated care, and convenience—and who prefer having all their care managed under one system, from doctor visits to prescriptions.
