Congratulations! You decided to change your residence. This can help you expand your social circle, establish new friendships and connect with people to help enrich any aspect of your personal or professional life. If your move affects your medical eligibility, this qualifies as a life change event.
 
Event Checklist
 
  • Contact your medical provider: Discuss your options

For medical, you may enroll in a plan option available in your new location if your current plan is not, enroll in a newly available plan option (regional HMO), if you are enrolled in an HMO, and your covered dependent moves, you may enroll yourself and your eligible dependents in another option that provides coverage for you and your eligible dependents. Your move will have no impact on your vision benefits and there's no impact on dental benefits (unless you are enrolled in a Dental HMO). 

For Flexible Spending Accounts - No changes are permitted for Health Care FSA. If your move affects Dependent Care FSA expenses you may enroll, increase/decrease your contributions for the remainder of the plan year, or stop participating. Please contact 408.952.8919 or Benefits@vta.org.

  • Medicare: You may cancel your, your spouse's, or your dependent's medical, dental, vision coverage as applicable. You can decrease/revoke your contributions for the remainder of the plan year for Health Care FSA. No changes are permitted for Dependent Care FSA.

Contact the HR Department - Benefits Unit for a Benefit Change/Enrollment Form, WITHIN 60 DAYS OF THE QUALIFYING EVENT, to update your benefit information.
