Eligibility & Enrollment


Employee Eligibility

Any active, regular, full-time employees working a minimum of 30 hours per week are eligible.

Dependent Eligibility

  • Your legal spouse or domestic partner;
  • Your dependent children (including your step-children and legally adopted children) up to age 26;
  • Dependent children of any age who are incapable of self-support due to a disability
When Does My Coverage Go Into Effect?

Benefits become effective first of the month following 30 days of employment. Current employees can make enrollment decisions only during the annual Open Enrollment period. All enrollments are completed on the EASE Portal.

The company has a standard effective date of November 1st. each year, which is processed by the Annual Open Enrollment period in the month of October. For New Hires throughout the year coverage is effective the 1st of the month following 30 days of employment. 

You have 30 CALENDAR DAYS from your date of hire to enroll in benefits.

Changes in Benefit Elections

You may not change your benefit elections during the year unless you have a qualified family status change. If you have one of the below-qualified family status changes during the year, please contact HR@ursusinc.com within thirty (30) days of the family status change.

Qualifying events include (but aren't limited to):
  • Marriage or divorce
  • Change in spouse's employment status
  • Birth or adoption of a child
You must notify HR@ursusinc.com or HR Dept within 30 CALENDAR DAYS of the qualified life event to make changes to your benefit elections.

Enrolling in Benefits

EASE is our online self-service benefits administration system. With just a few clicks you can enroll and make changes to your benefits, designate beneficiary(ies) for Life/AD&D insurance, find plan summaries, evidence of coverage booklets, and more. Be sure that all of your dependent information is accurate and complete. If you are enrolling new dependents, you will need their Social Security numbers and dates of birth.
