When can I make changes to my benefits?


The benefit choices you make upon initial enrollment or during the Annual Enrollment period will remain in place until the next Annual Enrollment or change in family status, also known as a qualifying life event.

For mid-year changes, you have 30 days from the date of a qualifying life event to request a change to your coverage. Benefit changes made as a result of a family status change must be consistent with the event. Proof of family status change is required under the benefit plan.

NetBenefits is our online self-service benefits administration system. Be sure that all of your dependent information is accurate and complete. If you are enrolling new dependents, you will need their Social Security numbers and dates of birth.

DOCUMENTATION REQUIRED

 

Marriage

 

Add dependent(s) to existing coverage including spouse’s children

 

Copy of marriage certificate AND if adding children, copy of birth certificate

 

Divorce

 

Drop former spouse from all coverage

 

Copy of final divorce decree

 

Birth/Adoption/Placement of a Child

 

Add new dependent to existing coverage and changes to FSA

 

Copy of birth certificate OR
Proof of birth letter from hospital OR
Copy of adoption or placement agreement

 

Employee and/or dependents gain other group coverage

 

Drop dependents
Drop benefits

 

Proof of other group coverage for each individual being dropped

 

Employee and/or dependents lose other group coverage

 

Add self and/or spouse and dependents

 

Proof of loss of group coverage AND marriage certificate and birth certificate(s) for newly added dependents
