Leaves of Absence

 

TRISTAR is CEI’s Leave of Absence Administrator

There may be times when you need to take a leave of absence for any of the following reasons:

  • A serious health condition
  • For a family member’s serious health condition
  • Maternity/Pregnancy
  • Parental Bonding (Birth, Adoption or Placement of a Foster Child)
  • Military Family Leave (Qualifying Exigency or Care of an Injured Servicemember)
  • Military service or active duty
  • ADA Accommodation
  • Personal Leave

How Do I Report a Leave?

*Notify your direct manager to report your absence.

Contact: Tristar at 1-844-702-2352 to report your leave.

  • Please have the following information available when you report your claim/leave:
    • Reason you are out of work (medical condition/symptoms)
    • Your last day worked, first day absent from work, and anticipated return to work date.
    • After you submit the leave request, a specialist will reach out to you within two business days to discuss the leave and send a leave packet.
    • Submit your required documentation to Tristar’s secure email ICSFax@tristargroup.net within 15 days to approve or deny the leave.
    • Check the status of your claim/leave by calling your Leave Specialist at 1-844-702-2352.


If you have a CEI work email,
you can report a leave online when you visit cupertino.ess-absencetracker.com, and follow these instructions:

  • Register a new account with your work email. You will receive an email from AbsenceSoft to create a password.
  • After you create a password, log in to AbsenceTracker and click on the icon “Leave Request.”
  • Submit the leave request, and a leave specialist will contact you after two business days to discuss the leave.
  • Submit your required documentation to Tristar’s secure email ICSFax@tristargroup.net within 15 days to approve or deny the leave.
  • Check the status of your claim/leave online or by calling your Leave Specialist at 1-844-702-2352.

Employees in the State of California

The California State Disability Insurance (SDI) program provides short-term Disability Insurance (DI) and Paid Family Leave (PFL) wage replacement benefits to eligible workers who need time off work.  You may be eligible for DI if you are unable to work due to non-work-related illness or injury, pregnancy, or childbirth.  You may be eligible for PFL to care for a seriously ill family member or to bond with a new child.
