Basic Life and Accidental Death & Dismemberment (AD&D) Insurance help provide a foundation of financial protection for your family if something happens to you. All full-time employees are automatically enrolled in the Mutual of Omaha Basic Life and AD&D plan. The Basic Life and AD&D plan is entirely paid for by Advanced Systems Group.

Basic Life provides a cash benefit of $10,000. Basic AD&D provides an additional benefit if your death is the result of a covered accident. It also pays benefits for loss of limb, hearing, speech, sight, and other serious losses resulting from an accident. See your policy documents for details of coverage.

Voluntary Coverage

For additional coverage, you can purchase Voluntary Life and AD&D insurance for you and your family. You must enroll in coverage for yourself in order to cover your spouse or children. If you don’t enroll in Voluntary Life when it’s first available to you, or elect an amount over the Guaranteed Issue, you maybe required to complete an Evidence of Insurability (EOI) form. Please visit Mutual of Omaha to access the EOI form. Once completed, you will then receive an approval or denial letter from Mutual of Omaha.
