Mutual of Omaha Disability Income Protection

Both Short and Long Term Disability are Voluntary plans and if elected, are paid 100% by you, the employee.

Voluntary Short Term Disability (STD)

If you cannot work because of an illness or
injury for more than 7 days, Advanced Systems Group offers a Voluntary short term disability plan (STD) that helps replace a portion your income.

After 7 days of disability, you will be eligible to receive a weekly benefit based on a percentage of your weekly salary. The amount of your benefit is based on your weekly income in effect just prior to your disability date. Your weekly salary is defined as your pre-disability earnings.
Weekly benefits may replace up to 60% of your salary to a maximum benefit of $2,500 per week for up to 25 weeks.

Note: For CA Employees: SDI provides disability and Paid Family Leave (PFL) benefits equal to 60% of the employee’s base period earnings.  If you elect to enroll in the Voluntary STD plan, your short-term disability benefits will be coordinated with CA SDI and CA SDI will pay first with Sun Life’s benefits covering any additional funds up to 60%. Voluntary Long Term Disability (LTD)

If you continue to be disabled for 180 days, you
may be eligible to receive a monthly benefit based on a percentage of your monthly salary. The amount of your benefit is based on your monthly income in effect just prior to your date of disability. Your monthly salary is defined as your pre disability earnings.

The LTD plan does not cover any disabilities caused by or resulting from a pre-existing condition. You have a pre-existing condition if you received medical treatment, consultation, care or services including diagnostic measures, or took prescribed drugs or medicines in the 3 months just prior to your effective date of coverage; or you had symptoms for which an ordinary prudent person would have consulted a health care provider in the 3 months just prior to your effective date of coverage; AND disability begins in the first 12 months after your effective date of coverage.

LTD benefits can replace up to 60% of your salary to a maximum benefit of $15,000 per month.
