Active, regular, full-time employees working a minimum of 30 hours per week are eligible to enroll in benefits. Benefits are effective on date of hire.
Dependent Eligibility
- Your spouse or domestic partner; or
- Your dependent children (including your step-child, foster and legally adopted child) up to age 26; or a child for whom a Qualified Medical Child Support Order (QMCSO) is in place
- Any dependent child who reaches the age limit and is incapable of self-support because of a mental or physical disability.
Making Changes During the Plan Year
In most cases, you may only make changes to your benefits during Open Enrollment. However, if you have a “qualified life event,” you may make changes to certain benefits, as defined by the plan documents, related to that event. Qualified life event changes are effective on the date of the event as long as you make the request and submit the necessary documentation within 30 days of the event.
Common Qualified Life Events
- Marriage;
- Divorce or legal separation;
- Birth, adoption or placement for adoption of an eligible child;
- Loss of life of a spouse or covered child;
- Change in your or your spouse’s work status that affects benefits eligibility (for example, starting a new job, leaving a job, changing from part-time to full-time, starting or returning from an unpaid leave of absence, etc.)
- A significant change in your or your spouse’s health coverage attributable to your spouse’s employment; A change in your children’s eligibility for benefits;
- Becoming eligible for Medicare or Medicaid during the year; and/or
- Becoming eligible for domestic partner status
When and How You Enroll
New hires can enroll at any time within 30 days of their hire date. Benefits become effective on date of hire. Current employees can make enrollment decisions only during the annual Open Enrollment period. Current employees may only enroll during the designated Open Enrollment period or when experiencing a qualifying life event.
