FOLX Health
FOLX Health offers comprehensive services for the LGBTQIA+ community. With our diverse network of LGBTQIA+ specialized providers, we deliver affirming and joyful healthcare experiences that celebrate your identity and honor your choices. In addition, FOLX offers support for everyone who works with or loves someone in the community including parental support for parents of LGBTQIA+ youth as well as educational content and training modules on how to support friends, co-workers, and direct reports.
What you need to know:
All employees, spouses/domestic partners, and adult dependents (18+) are eligible for a FOLX Health membership to access all non-clinical services at no cost. All employees, spouses/domestic partners, and adult dependents (18+) enrolled in a Premera health plan will have access to FOLX's clinical services through insurance. All other employees are eligible for non-clinical services and can access clinical services at FOLX’s cash pay rates or through their own medical plan, if available.
FOLX offers a comprehensive range of services specifically designed by and for LGBTQIA+ individuals, including gender-affirming care, sexual and reproductive health services, mental health support, virtual primary care, fertility education, ally support and education, and more.
As a FOLX member, you get access to:
- Book and see an LGBTQIA+ expert clinician within a few days for gender-affirming care, virtual primary care and preventive care, sexual and reproductive health care, and mental health care
- Message our team of Member Navigators, who can help you with referrals to in-person LGBTQIA+ affirming providers and resources
- Explore the FOLX Community Platform, where you can find LGBTQIA+ content and join discussions with fellow community members
- Join a support group
- Get letters for surgery or name and gender marker changes

Premera
Premera offers coverage for the treatment of gender dysphoria. Benefits are provided for medically necessary surgical services, including facility and anesthesia fees, and are available for specialized surgical procedures and other related services, depending on your plan.
What you need to know:
This benefit covers you, your spouse/domestic partner, and enrolled dependents. You must be age 18 or older, capable of making an informed decision about consenting to treatment, and diagnosed with gender dysphoria. Kaiser
As a Kaiser Permanente member, you have access to a broad range of gender-affirming care and services. Kaiser physicians, care teams, facilities, and health plans are all part of one connected team - the same team as our mental health clinicians, pharmacists, and lab technicians. That makes it easier for you to get the support you need for a healthy gender-affirming care journey - and a positive, empowering experience.
